One Way Reservation Form

To make your one way reservation, in coordination with a commercial or private flight, fill out the form below or call us at 231-539-7005. We will email or call you with a quote and to confirm your reservation. Please keep in mind that if you send this form after our business hours we will confirm it the following day. Late night transportation is based on Availability ONLY.

REGARDING CREDIT CARD PAYMENTS:

We accept Visa, Mastercard and Discovery.
We DO NOT accept American Express (AMEX) at this time.
Your payment will be NOT be processed until the day of service!

I Acknowledge The Following: By checking this box you give us permission to debit your account for the amount indicated on or after the indicated date. This is permission for a single transaction only, and does not provide authorization for any additional unrelated debits or credits to your account. I authorize Mackinaw Shuttle (1395 US-31 HWY, Pellston, MI. 49769 231-539-7005 info@mackinawshuttle.com) to charge the credit card indicated in this form according to the terms outlined here. This payment authorization is for the goods/services described above, for the amount indicated on the follow up email only, and is valid for one time use only. I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in this form. We have a 24 hour cancellation/no show policy. If you cancel your reservation within 24 hours of your scheduled arrival, the cancellation fee is ½ of your agreed upon rate.
